Dave, insulating slab goes under the dense concrete slab which for efficiency still absorbs the heat from fire (unless the hearth had heat absorbing thickness just of the firebricks size.) Starting from 5cm (2 inch) thickness for vermiculite slab will be good. No need for aluminum foil between these two, only use it between firebricks and wet concrete application.
The foil is used for two reasons, it stops concrete bonding with firebricks and it won't allow firebricks to take water from concrete when it is put on or curing.
