Max size for clay-oven sand form??????

I am looking at building a 4 foot by five foot clay oven by commission. Does any one know the going price for building an oven of this size if the materials are all paid for?
Would a sand form work? I have experience with sand forms and I am more comfortable with this method rather than the sapling form. I am also thinking of using bricks for an arch door to provide a more durable entry, any suggestions on incorporating this? Door ideas? I also am wondering about the slab.
Someone poured a slab on top of a rock structure with no re-bar, what should I do to reuse this base but make sure the slab is strong enough, pour an additional one on top? Thank you for your support and thoughts.
